Cerebral Pathology

Summary
Patients with ischemic and hemorrhagic stroke are at high risk of systemic
complications. The causes leading to gastrointestinal bleeding in stroke are
unknown, and gastroduodenal ulcers caused nonspecific response to any
"aggression", including the nervous system lesions, are so-called stress ulcers [15].
However, only in one of several studies that describe post stroke complications
commented gastrointestinal bleeding, and in others they are not described as stroke
complications [4].
The peculiarity of the clinical course, features and diversity occurrence
symptomatic ulcer mechanisms indicate the need to isolate this pathology in a
particular group is different recognition difficulty and high frequency of
complications (perforation, bleeding) [9].

graphically, catecholamine, reflex dystrophy, tropism of the secretary apparatus of
the stomach.

N.K.Bogolepov [7] in the analysis result 975 autopsy of patients who died from
cerebrovascular accidents, set at 150 petechial hemorrhages, erosions and ulcers in
the small gastro duodenal area. These mucosal defects were observed mainly when
the pathologic nidus area captured diencephalon and barrel where localized
vasomotor centers-tropic innervations.
A.A.Yarosh all gastrointestinal disorders detected in 252 patients with
hemorrhagic and ischemic stroke, divided into hemorrhagic, dystrophic and
dyskinetic options [3]. For the first variant is characterized by vomiting of blood.
This is due to an increase in vascular permeability of the stomach wall and the
small intestine, due to disturbances of the vascular tropic innervations in
connection with pathological processes in the brain. The second embodiment is
characterized in that when the patient's life radio graphically in the stomach and
small intestine are marked defects. At autopsy revealed acute visceral congestion
with hemorrhagic foci in the gastric mucosa (GM). Histology revealed patchy
necrosis of the surface layers of the mucosa. For the third variant is characterized
by dysmotility and secretary functions. In the mechanism of stress ulcer attended
by a variety of factors, each of which can hardly be called decisive, and here,
apparently, plays the role of a coordinated simultaneous effect on the gastric
mucosa. Great importance is attached and tropic processes in the body on the basis
of the main suffering [22].
In connection with the public morphological manifestations steroid ulcers with
lesions developing under the action of super strong stimuli - stress analyzes
changes in the gastric mucosa from the point of view of possible interaction of
glucocorticoids and neurotransmitters of the sympathetic nervous system. These

data suggest that the formation of ulcers associated with trophic disorders and
mucosal regeneration delay resulting stress influences, in particular of
glucocorticoids and catecholamine [16].
The majority of patients in the acute stage of subarachnoid hemorrhage caused
by rupture of an arterial aneurysm of the brain, the hypothalamus was found to
defeat him and autonomic centers [4]. Neurogenic value and, in particular,
hypothalamic effects in the mechanism of ulcerative dystrophic lesions gastro
duodenal validated by many authors [4,13]. At the same time the realization of
neurogenic effects hitherto not completely understood. There is an idea that
pathological impulses, leading to malnutrition, is delivered to the stomach via the
vagus nerves [12]. This is supported by the occurrence of hemorrhages, erosions
and ulcers in the gastric damage and stimulation of the vagus nerve [10, 21], and
under the influence of injection of large doses of parasympathetic agents [14, 22].
Based on numerous experimental data obtained mainly in small animals, SV
Anichkov et al [2] suggest that "reflex dystrophy", including the hypothalamus as
an intermediary are implemented by the sympathetic nerves. However, stimulation
of sympathetic nerves lowers the secretion of hydrochloric acid gastric glands and
enhances mucus secretion [3,4], which is known to inhibit ulcer formation. Widely
known as the concept of the implementation of the ulcerogenic effects of
hypothalamic system through the hypothalamus - pituitary - adrenal cortex [9,10].
It is believed that the destructive changes in the coolant are realized by
activation of adrenergic effects [17].
Shows the role of the sympathetic nervous system in regulation of metabolism
stomach [18] and ulcerogenic effect of excess catecholamines [5].Literature data
on changes of acid in the stomach and the mechanism of its violations during stress
contradictory and need to be clarified. Some authors indicate a decrease in the
